Type 1 Diabetes usually occurs in children and young adults, who must have insulin delivered by injections or a pump in order to survive. Type 1 Diabetes may account for 5% to 10% of all diagnosed cases of diabetes. Risk factors include autoimmune, genetic, and environmental factors.

Type 2 Diabetes : Type 2 Diabetes ( previously known as non insulin-dependant diabetes mellitus   ( NIDDM ) or adult-onset diabetes ) develops gradually, and is usually caused by a combination of impaired secretion of insulin and reduced sensitivity of the body's cells to insulin ( insulin resistance ). As a result, blood glucose levels become elevated.

Type 2 Diabetes usually occurs in adults, however it is increasingly being diagnosed in people at younger age, even in childhood and adolescence.

Type 2 Diabetes may account for about 90% to 95% of all diagnosed cases of diabetes, and the prevalence is rising at an alarming rate throughout the world.

This is believed to be due to increases in longevity, sedentary lifestyles and a dramatic upsurge in obesity.

High Blood Sugars can be controlled.

Therapeutic lifestyle management such as following a careful diet and exercise program, and losing excess weight may help, though oral medications are often necessary. Risk factors include old age, obesity, a family history of diabetes, prior history of gestational diabetes, impaired glucose tolerance, physical inactivity, and race/ethnicity.

African Americans., Hispanic / Latino Americans, American Indians, and some Asian Americans and Pacific Islander are at particularly high risk for Type 2 Diabetes.

Impaired Glucose Tolerance ( IGT ) : Impaired Glucose Tolerance is an intermediate state between normal blood glucose control and type 2 diabetes. IGT is an early sign that a person's carbohydrate metabolism is impaired. It carries a high risk of progressing to Type 2 Diabetes.

IGT ( and Type 2 Diabetes ) result from a combination of impaired secretion of insulin and reduced sensitivity of the body's cell to insulin ( insulin resistance )

Gestational Diabetes : Gestational Diabetes is a form of glucose intolerance diagnosed in some women during pregnancy, but usually disappeared after the mother gives birth. Treatment is required to normalize blood glucose levels to avoid complications in the infant.

Gestational Diabetes occurs more frequently among African Americans, Hispanic / latin Americans, and Native Americans. It is also more common among obese women and women with a family history of diabetes.

Secondary Diabetes : Secondary Diabetes can result from other conditions such as specific genetic syndromes, surgery, drugs, malnutrition, infection, and other damage to or diseases of the pancreas.

How Is Diabetes Be Diagnosed

Type 1 : Patients with Type 1 Diabetes usually develop symptoms over a short period of time, and the condition is often diagnosed in an emergency setting.

Urinalysis of an acutely ill Type 1 Diabetes patient will detect high glucose levels, and high levels of ketones.

Ketones are produced by the breakdown of fat and muscle, and they are toxic at high levels. Ketones in the blood can cause a condition called " acidosis " ( low blood pH ). Blood glucose levels are also high.

Type 2 : Patients with Type 2 Diabetes develop symptoms over a longer period of time. Type 2 diabetes is diagnosed when:

The blood glucose is 126 milligrams per deciliter       ( mg/dl ) or higher on two occasions after fasting  (abstaining from food) for 8 or more hours; or...

The blood glucose level is 200 mg/dl or higher at any time between meals with symptoms of diabetes, such as increased thirst, urination, and fatigue; or...

A blood glucose level drawn two hours after drinking a 75-gram glucose solution is 200mg/dl or higher.

Diabetes : Risk Factors and Preventive Measures

Risk factors are characteristics that can predispose you to developing a condition or disease. Just because you have one or more risk factors does not mean you will get diabetic. Risk factor for Type 1 Diabetes are not as clearly defined as for Type 2 Diabetes.

Type 1 Diabetes risk factors include...

• Autoimmune disease, where the body mistakenly attacks the insulin-producing cells

Type 2 Diabetes risk factors includes...

What Are The Problems Associated With Diabetes?

Many people with diabetes eventually develop complications, especially if it is not controlled well. Even if the diabetes is controlled well, complications can still occur. The good news is that short-term complications can be easy to fix and that proper treatment of diabetes usually delays complications.

The bad news is that long-term complications are difficult to control and can cause someone to die earlier than normal. Keep in mind as you read this section that there are some individuals who have had diabetes  for over 50 years and have experienced few complications

This is how Diabetes becomes a Silent Killer

1. Hypoglycemia : A very common short-term complication of diabetes. This complication can occur during treatment if the amount of food eaten and the amount of insulin taken are not balanced properly. Hypoglycemia is more common in people with Type I diabetes but can occur in people with Type 2 Diabetes who take sulfonylurea drugs.

Sulfonyurea drugs increase the production of insulin from cells in the pancreas. If untreated, hypoglycemia can cause seizures, which are involuntary muscle movements and/or decreased awareness of the environment due to overexcitement of nerve cells in the brain. Ketoacidosis  is an example of a short-term complication of diabetes.

2. Infection : High levels of sugar in the blood make it more difficult for the body to fight against infections. This can lead to infections of the urinary tract, which is the part of the body that deals with the formation and excretion of urine (pee). To excrete means to release from the body as waste.

3. Eye Problems : Eye problems can develop as a complication, such as retinopathy (also known as diabetic retinopathy), which is damage to the retina and the blood vessels that serve it. The retina is an area at the back of the eye that is sensitive to light.

Blood vessels are tube-shaped structures that carry blood to and from the heart. Both large and small blood vessels also start to break down quicker in people with diabetes mellitus. Blood may also have a difficult time moving throughout the body as a result of diabetes mellitus.

4. Peripheral Neuropathy : Damage to nerve fibers outside of the brain or spine. Peripheral neuropathy can cause a gradual loss of sensation starting at the hands and feet, which sometimes moves up the arms and legs.

Loss of feeling and poor blood circulation makes the body more susceptible to ulcers (open sores) and gangrene (tissue death due to poor blood supply or infection of a wound.). Peripheral neuropathy can also cause dizziness when standing up as well as impotence in men.

5. Problems With Sexual Function : Diabetes may cause difficulties during sexual intercourse, such as erectile dysfunction, soft erection & etc.

6. Kidney Damage : Kidney damage can occur as a complication of diabetes mellitus, which can lead to kidney failure.

7. Heart Disease : Another complication of diabetes mellitus is a higher risk for atherosclerosis, which is a narrowing of arteries (blood vessels that carry blood away from the heart). Atherosclerosis can cause damage to large blood vessels, which is a major cause of stroke and coronary artery disease.

A stroke is a burst artery (a type of blood vessel that carries blood away from the heart) or a blockage of an artery in the brain.

Coronary artery disease is a narrowing of coronary arteries, which supply the heart with blood. The narrowing of coronary arteries causes heart damage.

People with diabetes also have a greater chance to have increased levels of cholesterol, which can speed up the development of atherosclerosis. Cholesterol is a waxy, fatty substance found only in animal tissues.

8. Other Complications : High Blood Pressure, Heart Disorders, Strokes and Cataracts are additional complications associated with diabetes mellitus. Cataracts is a darkening of the lens in the eye. The lens is an organ located between the colored part of the eye, that bends light as it enters the eye.

A very important issue for people with diabetes mellitus to be concerned about is good foot care. This is because one complication of diabetes mellitus is ulcers (open sores) on the feet. In severe cases, ulcers can develop into gangrene. Gangrene is death of a tissue, usually due to a loss of blood supply. If a foot sore develops, you should see the doctor immediately.

Fact : Complications of diabetes

Heart disease and stroke

Heart disease and stroke account for about 65% of deaths in people with diabetes.
Adults with diabetes have heart disease death rates about 2 to 4 times higher than adults without diabetes.

The risk for stroke is 2 to 4 times higher and the risk of death from stroke is 2.8 times higher among people with diabetes.

In women with diabetes, deaths from heart disease have increased 23 percent over the past 30 years compared to a 27 percent decrease in women without diabetes.

Deaths from heart disease in men with diabetes have decreased by only 13 percent compared to a 36 percent decrease in men without diabetes.

High blood pressure

About 73% of adults with diabetes have blood pressure greater than or equal to 130/80 millimeters of mercury (mm Hg) or use prescription medications for hypertension.

Blindness

Diabetic retinopathy causes 12,000 to 24,000 new cases of blindness each year making diabetes the leading cause of new cases of blindness in adults 20-74 years of age.

In people with type 1 diabetes, therapy that keeps blood glucose levels as close to normal as possible reduces damage to the eyes by 76% (New England Journal of Medicine, September 30, 1993).

Experts believe that these results can also be applied to those with type 2 diabetes.

Mexican Americans are almost twice as likely and non-Hispanic blacks are almost 50% as likely to develop diabetic retinopathy as non-Hispanic whites.

Kidney disease

In 2002, a total of 153,730 people with ESRD due to diabetes were living on chronic dialysis or with a kidney transplant.

In people with type 1 diabetes, therapy that keeps blood glucose levels as close to normal as possible reduces damage to the kidneys by 35% to 56% (New England Journal of Medicine, September 30, 1993). Experts believe that these results can also be applied to those with type 2 diabetes.

Non-Hispanic blacks are 2.6 to 5.6 times as likely to suffer from kidney disease with more than 4,000 new cases of ESRD each year. Mexican Americans are 4.5 to 6.6 times more likely and American Indians are 6 times more likely to suffer from kidney disease.

Nervous system disease

About 60% to 70% of people with diabetes have mild to severe forms of nervous system damage. The results of such damage include impaired sensation or pain in the feet or hands, slowed digestion of food in the stomach, carpal tunnel syndrome, and other nerve problems.

Almost 30% of people with diabetes aged 40 years or older have impaired sensation in the feet (i.e., at least one area that lacks feeling).
Severe forms of diabetic nerve disease are a major contributing cause of lower-extremity amputations.

Amputations

More than 60% of nontraumatic lower-limb amputations occur in people with diabetes.

In 2002, about 82,000 nontraumatic lower-limb amputations were performed in people with diabetes.

The rate of amputation for people with diabetes is 10 times higher than for people without diabetes.

Mexican Americans are 1.8 times as likely, non-Hispanic Blacks are 2.7 times as likely, and American Indians are 3 to 4 times as likely to suffer from lower-limb amputations.

Amputation rates are 1.4 to 2.7 times higher in men than women with diabetes.

Diabetes is the fifth-deadliest disease in the United States. Since 1987 the death rate due to diabetes has increased by 45 percent, while the death rates due to heart disease, stroke, and cancer have declined.

Based on death certificate data, diabetes contributed to 224,092 deaths in 2002. Studies indicate that diabetes is generally under-reported on death certificates, particularly in the cases of older persons with multiples chronic conditions such as heart disease and hypertension.
